Procedure

Brazilian Butt Lift

Anatomical fat transfer planned around proportion and projection — never volume for its own sake.

About

What this procedure is

A Brazilian Butt Lift removes fat from areas of excess and re-deposits it to enhance contour and projection. Our approach prioritizes safe technique under ultrasound guidance, anatomical landmarks, and proportions designed for the individual frame.

A BBL is fundamentally a fat-transfer operation, and its outcome depends as much on the donor harvest and contouring of the surrounding waist as on the volume placed. Proportion is the primary design constraint — not maximum projection.

Our technique places fat in the subcutaneous plane only, with intra-operative ultrasound used to confirm placement and protect the gluteal vasculature. This approach is consistent with current safety guidance from the major plastic surgery societies.

Planning

How Dr. Gelb plans the procedure

  • Proportion and projection planned to your skeletal frame and waist line.
  • Subcutaneous fat placement performed under ultrasound guidance for safety.
  • Refined contouring of the surrounding waist, back, and outer thighs.

Frame analysis precedes any volume planning: shoulder width, waist position, hip break, and lower-back inscription. Volume is then calibrated to that frame rather than to a target number of cubic centimeters.

The right plan is determined in consultation, based on your anatomy, history, and goals — not from a template.

Technologies & Techniques

Tools matched to anatomy

Technology is selected when it improves the result — never by default. Tools serve the plan.

Safety & Evidence

A safety-first standard of care

  • Subcutaneous-only placement under ultrasound guidance.
  • Board-certified anesthesia in an accredited facility.
  • Conservative volumes — over-grafting is declined.
Recovery

Evidence-based recovery

Patients sit and sleep with care for several weeks to protect fat graft survival. Compression, light walking, and structured follow-up support take. Recovery timelines and results vary. The plan, expectations, and timeline outlined here are general — your specific course is determined at consultation.

Days 1 – 7
Sitting protocol per your specific plan. Compression, light walking, and graft protection are the priorities.
Weeks 2 – 6
Lymphatic drainage and gradual return to desk work. Sitting protocols ease per your team's guidance.
Months 2 – 6
Donor-site contour settles; transferred fat that survives behaves like the rest of the body's fat.

FAQs
How is your BBL approach focused on safety?+

Fat is placed in the subcutaneous plane only, under ultrasound guidance, with attention to anatomical landmarks consistent with current safety guidelines.

How much fat survives long-term?+

Graft retention varies by patient. A portion of the transferred fat is typically reabsorbed in the first months; what remains is generally stable long-term.

When can I sit normally?+

Sitting protocols are reviewed in detail at your pre-operative appointment and depend on your specific plan and graft volume.
